| Responsibilities: | As a Home Health Aide you will be trained to assist clients with personal care and activities of daily living in all Agency Programs. The home health aide works as a member of the health care team of the Agency following a plan of care established and supervised by the nurse or therapist. In the Adult Day Health, the home health aide also assists the professional program staff in implementing the program services and meeting the needs of individual program participants.

| Qualifications: | Must have successfully completed the required training program and possess Home Health Aide Certification. Must be able to read, write and speak English adequately to perform job functions as well as a minimum of one year experience working with adults in a health care setting.
