Registered Nurse Positions

Homecare/Hospice Registered Nurse - All Shifts
Utilizing the nursing process, plans for coordinated home health and hospice services for individuals and their families. Provides appropriate preventive, palliative and rehabilitative care through assessment, teaching, counseling and direct care to client and/or family members. Coordinates care with other community resources as needed. Coordinates care with other team members and attends Hospice Interdisciplinary Team Meetings as indicated.         
Telehealth Nurse - RN
 In this role you will be responsible for managing our Telehealth program by providing direct supervision of our program;, triaging telephone calls from staff, patients and physicians; and providing community visits as needed.  

Home Health Aide Positions

Home Health Aide I
As a Home Health Aide you will be trained to assist clients with personal care and activities of daily living in all Agency Programs. The home health aide works as a member of the health care team of the Agency following a plan of care established and supervised by the nurse or therapist.

Rehab Positions

Occupational Therapist -- All Shifts
As a Registered Occupational Therapist you will provide direct care to our clients under a physician's orders. In addition, you will evaluate our clients and design and provide a therapeutic program in coordination with our other clinician disciplines. You will also be responsible for the teaching and supervision of our Home Health Aides who are assisting with the therapy service delivery as well as our Therapy Assistants as appropriate.  
Physical Therapist -- All Shifts
As a Physical Therapist you will provide direct care to our clients under the orders of a physician. You will also evaluate clients and design and provide a therapeutic program with realistic goals with attention to improving functional outcomes.   
Weekend Physical Therapist
 As a licensed Physical Therapist you will provide physical therapy services under physician's orders and in coordination with other staff disciplines. In addition, as a Physical Therapist, you will be responsible for the teaching and supervision of Home Health Aides who are assisting with therapy service delivery and personal care to clients and Physical Therapy Assistants as appropriate.   

Administrative Positions

Clinical Coordinator - RN
Oversees the coordination and scheduling of effective and efficient delivery of homecare/hospice services, assuring quality care for clients and/or family members. Provides leadership and support to our clinical team and works closely with the Director of Clinical Services and Rehab Supervisor to assure optimal staff productivity. " Assists with troubleshooting daily clinical issues and clinical problems with the Director of Clinical Services and Rehabilitative Supervisor. " Triages phone calls with clinical questions; notifying appropriate clinicians regarding client issues, taking phone orders, documenting phone orders and conversations, and relaying messages to the appropriate clinicians. " Performs home visits for orientation, evaluation, observation, supervision and instruction of clinical staff and/or provides direct care as necessary. " Assigns and schedules all new clinical admits and oversees the schedule, assuring that all patients are seen as ordered. " Provides HHA supervisory visits and assesses visits for rehab. The Clinical Coordinator is also responsible for administrative on-call back-up coverage as needed.
Intake Nurse - RN or LPN
This position holds primary responsibility for: 1. interfacing with customers who need information about Agency services and/or want to make a referral; 2. gathering all necessary information, completing required documentation; 3. coordinating with other staff to assure an effective efficient processing of referrals; and provides education to external customers about Agency services.  
Manager, Clinical Operations
This position holds responsibility for monitoring, collecting, trending and analyzing performance by: 1. monitoring compliance with Agency policies, regulations and best practice standards of care, 2. providing training and educational opportunities to assure clinical development, 3. monitoring risk through collection and analysis of key risk indicators, and 4. monitoring improvement in clinical outcome measurements. The ideal candidate will monitor compliance with Agency policies, regulations and best practice standards of care and performs regular record review of patients at or near discharge; perfoms OASIS integrity audits and transmits OASIS data to the state using the Agencys clinical software application; provides training and educational opportunities to Home Health and Hospice Staff; monitors risk through collection and analysis of key risk indicators; and, monitors improvement in clinical outcome measurements. The candidate will also develop competency in homecare coding and provide departmental back-up as necessary.
