Overview
Our
Hospice services are provided by an interdisciplinary team of highly
experienced professionals - nurses, aides, social workers, therapists,
spiritual care specialists, a medical director, and specially trained
volunteers. The patient and the family are always at the center of the
team’s attention. A first step in Hospice is developing a plan of
care. This involves the patient and his or her family, the referring
physician, and the entire care team. The goal of the care plan is to respect
each patient’s wishes, and those of family members, and to assure the the
highest quality end-of-life care. Putting the Hospice care plan into
operation on a daily basis requires the experience, quality, and dedication
of our staff. Over a third of our staff have been with
Parmenter for five years or more. Parmenter Hospice care is widely
recognized as superior by both professional referral sources and by families
who have shared the Hospice experience.
Hospice At Home
Some
people who are receiving Parmenter home health services experience a
life-limiting illness and can receive palliative care and Hospice care at
home. Palliative care centers on pain and symptom management and is
delivered by highly qualified clinicians. If an illness progresses to the
point where they decide not to pursue active, curative treatment any longer,
they can move to Hospice care at home, where the emphasis is on comfort
measures and maximizing the quality of life. These possible transitions,
from home health care, to palliative care, to Hospice care sustain a level
of trust, comfort, and continuity of care with Parmenter providing care as
the patient’s needs change over time.
